The mission of Bluegrass.org is to assist individuals and families in the enhancement of their emotional, mental and physical well-being by providing behavioral health, intellectual & developmental disabilities and substance abuse services. They recognize our responsibility to serve those who have limited options for meeting their needs. They plan with their communities, develop innovative programs to respond rapidly to needs and, as appropriate, help influence community priorities to ensure that individual and community service gaps are addressed.

Program areas at New Vista Communities

Management of oakwood facility - major accomplishments include:- oakwood celebrated its 50th anniversary on september 29, 2022. To commemorate the event, oakwood offered a photo booth, dj music, balloon sculpting, games, time capsule display, oakwood history slideshow, t-shirts for all clients and staff, prize drawings and lunch was served. Personnel from the cabinet for health and family services, division of developmental and intellectual disabilities, client parents and guardians, and local dignitaries were invited to attend.- the qualified intellectual disability professional (qidp) and social support coordinator (ssc) department increased positions from five to seven staff in january 2023. - the social support coordinators began an enrichment center community living classroom. The classroom opened on january 19, 2023, serving 22 oakwood individuals.- purchased ipads which are used for New employee orientation and facility training.- replaced over 30 outdated laptops with New ones.- replaced all 28 desktop computers used by nursing.- installed 2 video conferencing systems that will be used by the idt for hybrid meetings with the parents/guardians.- increased internet speed 5x for web-based patient care applications and emr access.- purchased New network switches for each building.- oakwood's readers to leaders were recognized by the chamber on april 4, 2023, and met with mr. todd, county judge executive, to map out the readers to leaders trail at pulaski county park. - tie dye t-shirts for disability pride month were made.- rights reduction project reviewed each individual's rights restrictions. Over 100 rights restrictions were restored through the process.- ddid requested a copy of oakwood's audit form to implement at other icf locations.- expressions tea and gifts (oakwood's off campus gift shop) re-opened on may 1, 2023. - oakwood's cafeteria re-opened on may 15, 2023.- garden center reopened to the public in april 2023.- expanded enrichment center offerings.- resumed special olympics with nine athletes competing in the track and field competition at southwestern high school.- reopened the swimming pool.- eku clinical psychology doctoral student started yearlong practicum during august 2023.- facilitated a trauma informed care survey related to secondary traumatic stress in partnership with New Vista trauma informed care (tic) team and the university of Kentucky center for trauma and children.- developed an oakwood trauma informed care (tic) team in july 2023. The first meeting to review tic survey results and set goals for the facility occurred on august 21, 2023. - two behavior/psychology department staff received certification as safety care instructors.- three behavior/psychology department staff received additional training on facilitating trauma informed care education for residential staff.- reinitiated comprehensive autism assessments for children through adults in the outpatient specialty clinic.- resumed residential skills system group, coping skills behavioral health therapy group.- director and assistant director of behavior and psychology services began participating in a monthly collaborative meeting with clinical and administrative staff from the state psychiatric hospitals, state intermediate care facilities, and the division of intellectual and developmental disabilities related to systems of care delivery services. - director of behavior and psychology services began participation in the pulaski county roundtable extended from the Kentucky judicial commission on mental health summit.- doctorate level clinician in the behavior/psychology department earned independent licensure as a psychologist and health services provider certification.- a New licensed professional counselor began employment in the behavior/psychology department.- a behavior technician earned certification as a registered behavior technician.- the behavior/psychology department implemented a comprehensive performance improvement project on peer-to-peer aggression reduction.
Management of bingham gardens facility - major accomplishments include:- bg is fully staffed as of march 2023. Vacant positions filled include dsps, ec supervisor, 2 home managers, housekeeping (sodexo), and maintenance.- the kitchens have been stocked with color-coded supplies. This has kept items being misplaced in other homes from happening. - home 6 opened.- 4 New admissions. 1 successful transfer from hazelwood to bingham gardens. - floors have improved with the purchase of the New floor cleaner. - secured 3 New volunteer opportunities at migration and refugee and 2 at the lords kitchen. - 1 New resident employment with sodexo working in linen room placing linens on shelves.- a group of residents signed up and attended qualifying practices for special olympics. - ec attendance increase from jan -59%, feb 79%, march 88%, to april 92%- updated safety equipment: training on proper/intended use.- quarterly trainings are ongoing.- New patio chairs kept well.- New light/fans on the back patio are a nice New addition. - the feat of louisville autism donated free admission tickets to the ky state fair to our individuals that included free parking and the ability to ride rides at no charge. The louisville bats donated 46 tickets for 8/23/23 and 26 tickets for sept. 6, 2023. Each ticket includes free admission, a hot dog, chips, and a drink. A client has gained a volunteer opportunity in the community at a local church. This will bring the total of (4) clients volunteering and (4) clients employed. - one bg client transitioned to an scl provider in july.- the outing schedule was revamped to diversify community integration opportunities for our residents. By doing this, they are getting exposed to more businesses and services in their local community. - p&a completed a keys to work training course for residents and employees. - lower vacancies rates for dsps. Bingham went from 16.28% in october to 1.92% end of march.- filled associate director of nursing at bingham.- bingham gardens medication refusal projectinitial committee completed with recommendations for follow-up.- meditech cloud migration testing (nursing functions within meditech) final check completed in august 2023.- lifevac anti-choking device added to emergency response policy.
Management of hazelwood facility - major accomplishments include:- hired a medical director after previous one retired. - created a New icf compensation structure with increases for employees.- lower vacancies rates for dsps. Hazelwood went from 30.93% in october to 11.86% at the end of march. - in january 2023, respiratory initiated a monthly performance improvement meeting, the minutes of which are placed in the sap minutes file on the p drive.- installed a 96tb network attached storage array for server backups. - installed cameras in the lee enrichment center.- installed a 1500va rackmount ups for the blade server. - license and support renewals for hazelwood server, bingham nvr/cameras, group home nvr, and server backup software.- intercom systems have been repaired and are functioning properly.- resident projects including charlie's charms, katrina's puppy love drive, paul's book sale, and josie's sweet treats have raised money for the crusade for children and humane society. The residents and staff are learning to sublimate items and will soon learn to use a laser cutting machine to expand their product possibilities.- therapy staff continue to facilitate the following clubs: yoga, book club, and joyful noise music club.- residents from each home participate monthly in the resident council.- residents had a st. patrick's day parade at each home.- performing arts showcase with residents at each home in october 2022. - board of health inspection was completed on 11/03/22 in the dietary department with passing scores of 99% in food service and 100% in retail (caf).- board of health inspection (routine) for the dietary department was completed on 6/22/23 scored 99%.- eco sure audit (third party auditor) was completed on 2/17/23 with a passing score of 100% in health and physical safety and 99% in food safety.- reach-in freezer for the dietary department was purchased, received, and installed. It replaced the unrepairable freestanding reach-in freezer.- cafe resumed full-time operating hours of 10am-4pm on 3/6/23.- improved work satisfaction by providing raises for nurses to match market value in the area.- filled key nursing positions (nurse case manager, 4 rns).- completed sams/learning objectives audit for all clients with follow up to fix any issues.- meditech cloud migration testing-compliance rn completed testing and communicated these to frankfurt.- infection control-nursing has monitored community covid transmission rates.- nursing has overseen coordinating covid outbreak testing, monitoring returned covid tests and the notification of positive testing to employees and government agencies for over 3 years. Outbreak testing has currently ended, but we will continue to monitor regulatory guidelines/recommendations and will act accordingly. - purchasing the enfit adaptors to facilitate a smooth transition to New enteral feeding tubing connectors.- coordinated purchasing and installation of safety locks for enteral feeding cabinets to keep residents safe.- coordinated presentation of td training for nursing.- hazelwood coordinated both influenza, rsv and covid outbreak situations at the main facility and the community homes.- coordinating influenza clinic with blumine health.- we have all nurses able to comply with the 2022-2023 nursing competencies which culminated last february 2023.- we hired almost to capacity at hw - a few part-time positions still open. - basically, with improved staffing, we have been making improvements by washing walls and getting floors back on project and maintenance schedules.- the pharmacy director brought specialty pharmacy medications in-house, increasing revenue for the pharmacy. - trauma informed committee surveys and developing committee in the works.- therapy has started multiple groups that are connected with community outings. This is ongoing.- in spring 2023, therapies started a friendship garden with resident and guardian assistance. - staff development and training have increased the number of ipads for staff to utilize to 30. We are now capable of using the ipads to support New employee orientation (neo) as well as the ongoing training needs.- we implemented a success track in brainier (our lms) to better track an employee's training during neo.- nursing retention & strengthen communication (nursing retention has been very good 2022/2023). Nurses' week recognition with food and activities daily during may 8th-12th, all shifts, all areas.- created a nursing department conference room where nursing committees, etc. Can meet. Interviews are done here, special recognitions, etc.right next to don's office which promotes greater communication more often.- initiated a protocol for adverse drug reactions that is proactive in assessing ahead of time for potential reactions in individuals. - staff and employee tb tests are at 100% for quarter.- launched four New aprns out into the community who had completed their training while working at hazelwood. This means four good nurses who love working with individuals with idd can now provide excellent care for folks with idd in the community.- infection control nurse is attending monthly apic meetings in louisville to stay abreast of any community issues related to ic so we can be always prepared.- a New transitions director has been selected (previous transitions coordinator).- a New transitions coordinator has been selected and will begin 9/5.- one hw client transferred to a private icf in august.- transitions has met 50% of our contract goal.- transitions has been selected to participate in the system of care collaboration workgroup facilitated by crystal adams with ddid. - hazelwood road paving project completed august 2023.- hazelwood fire alarm replacement project completed august 2023.- meditech cloud migration testing (nursing functions within meditech) final check completed in august 2023.- lifevac anti-choking device added to emergency response policy.- a New camera and nvr system for hazelwood. - the New dentist starts on september 18th. - the hazelwood enrichment center reopened on 8/14/23. Some classes and activities offered are arts and crafts, sensory activities, money management to identify coins/bills and safety awareness to name a few.- the feat of louisville autism donated free admission tickets to the ky state fair to our individuals that included free parking and the ability to ride rides at no charge.
