Agate Housing and Services

Emergency Services programs:shelter programs - shelter is offered at two locations, 510 and fcc, to up to eighty-six adults every night while we work with them to access Housing and support Services. In fiscal year 2023, 571 individuals were provided shelter between the two shelter locations.street outreach - this program provides the first contact for individuals living outside. Partnering with many local agencies and government offices, this intensive outreach connects people to shelter, Housing, and other Services. In fiscal year 2023, 1,144 unique unsheltered individuals worked with Agate's street outreach team.

Site-based programs:510 board and lodge this program provides permanent Housing and support Services to single adults. This program is maintained in a dormitory-style living situation with shared rooms, where residents are provided three daily meals. In fiscal year 2023, 150 individuals were served in this program.va beds this program provides 12 beds at our 510 board and lodge site for use by veterans. Our veteran residents typically stay in this setting for a few months while they utilize our case management Services to seek more independent settings for their next move. This program served 33 individuals in fiscal year 2023.park 7 Agate provides permanent Housing and support Services to residents at the park 7 building in downtown minneapolis, with 61 units of affordable Housing. The building is managed by trellis, an affordable Housing provider in the area, and all Services are provided by Agate. The program served 71 individuals in fiscal year 2023.food centre this program provides community meals 365 days a year and a food shelf program, operating under a "no questions asked" philosophy where no one is turned away. This program provides meal to the community at large, as well as feeding the residents at our 510 board and lodge facility and our shelter sites. Due to a fire in february 2022, we were unable to serve community meals until the final weeks of fiscal year 2023 and have instead run a smaller operation out of another kitchen to continue serving our board and lodge residents and shelter guests. In fiscal year 2023, we served 96,512 meals.day-by-day is an outpatient treatment program for substance use disorders, while also addressing mental health concerns through a day treatment model that includes sud assessments, mental health screening, group therapy, individual therapy, recreational activities, health education, and referrals for employment, education, health, and other needed Services. The program is now operated through a partnership with kyros.

Scattered site Housing programs:rapid rehousing programs - these programs help families and single adults transition quickly from shelter into Housing with supportive Services and short-term rental subsidies. 183 households were served, including 43 single adults, and 140 families including 303 children were served in fiscal year 2023.prevention programs - these programs work with families and single adults who are at risk of losing their Housing. The programs provide short- to medium-term financial support and case management Services to prevent homelessness. 69 households, including 40 single adults, and 29 families including 51 children were served in fiscal year 2023.next step assessments this program is funded to provide coordinated entry system Housing assessments for families in hennepin county. In fiscal year 2023, 762 assessments were completed with families this is more than double the number of assessments completed the previous year. Additionally, the team is now doing more work to follow up with individuals who they've assessed.supportive Housing programs - family and single adult supportive Housing programs provide long term, scattered-site Housing supportive Services for families and single adults who have experienced long-term homelessness and have a documented eligible disability. In fiscal year 2023, 75 families and 246 single adults received long-term case management. Minnesota Housing - Housing trust fund subsidies - rental subsidies are an important tool for helping some single adults and families maintain their Housing while they work toward self-sufficiency. These subsidies are provided through Minnesota Housing and serve up to 126 single adults and families.

Other programs:representative payee this program serves 408 individuals with managing their financial lives, including budgeting, as well as issuing the payment of rent, bills, personal needs, and ensuring that client benefits remain active. In fy23, Agate added a veterans rep payee program, which served 12 individuals with plans to grow.community engagement (education and advocacy) - this program advocates for systemic change. The program offers a day in the life, an experiential learning program that gives participants an opportunity to learn directly from people experiencing homelessness, education, and advocacy opportunities.birth certificate program this program supports people in acquiring needed person identification documents, primarily birth certificates. In fiscal year 2023, we facilitated the retrieval of 651 birth certificates for 606 people from 44 states/provinces/territories. This number is up again from last fiscal year's numbers, but still down from the pre-covid high from fy2019.handbook of the streets - in fiscal year 2023, 22,661 minneapolis handbooks of the streets were distributed (6,333 since the new book was published in march 2023). We distributed 11,113 st. paul books (10,366 since the new book was published in october 2022). We continue to broadly distribute street outreach trifolds.
