Home health aides - trained personnel to perform tasks dictated by a Nurse to Care for patients in the patient's Home. Examples are bathing, assist with dressing, ambulation and exercises dictated by a physical therapist. Includes Services to affiliates.
Home infusion - the Home infusion program is a joint program of the university of rochester medical center and Ur Medicine Home Care, Licensed Services, Inc. They deliver an integrated system that extends the university's clinical expertise into the Home Care setting. Ur Medicine Home Care, Licensed Services, Inc. Provides patient-centered infusion therapy and medical Care safely and effectively in the patient's Home. Home Care clinicians ensure that the patient and caregivers are taught effectively and are comfortable with the provided service and procedures.
Health Home Care management - consists of providers and community agencies that collaberate to provide Services to people with medicaid coverage who have a serious mental illness or two chronic conditions. It enables patients to be healthier and safer in the community.
