Program areas at Tender Care Human Services
Community habilitation: these Services provide the opportunity for the individuals to further their skills in the areas of mobility, social behaviors, self Care, basic safety, housekeeping, personal hygiene, health Care and financial management. These Services are provided by residential habilitation workers who are assigned to the individuals, and who work on a one-to-one regularly scheduled basis.
Medicaid service coordination: assist people with developmental disabilities in gaining access to necessary Services and support appropriate to their needs and life goals. It is provided by qualified service coordinators and uses a person centered process to develop, implement and maintain an individualized service centered plan (isp) custom made to meet the consumer and his or her family's needs.
Group day habilitation program: this program helps people with developmental disabilities gain vocational and employment skills to maximize their ability to lead satisfying, self-fulfilling and independent lives. The group day habilitation program is a community based model.
Waiver respite - the respite program provides planned short-term and time-limited breaks for families and other unpaid caregivers of children with a developmental delay and adults with an intellectual disability in order to support and maintain the primary caregiving relationship. Respite also provides a positive experience for the person receiving Care.
Family support Services - the family support Services programs provide a wide variety of Services directed at parents, siblings and other relatives that enhances a family's capacity to continue caring for a family member with a developmental disability living at home.
Other:bridges to health (b2h) - b2h is a high quality home-based clinical support and case management program that provides Services to children and youth diagnosed with serious emotional disturbance, developmental disability or medical fragility, and their caregivers, with an emphasis on family stability and educational success. Service recipients either are or have been in foster Care or in the ocfs juvenile justice system. The program has three arms, health Care integration (hci) which provides home and community base management, waiver service provision (wsp) which provides hands-on in-home support Services, and quality management which oversees the quality and the utilization aspects of the program in accordance with ocfs guidelines.omh-st. lukes&jcca - the home and community-based Services waiver (hcbs/omh) serves individuals, who if not for the hcbs waiver, would be admitted to institutional levels of Care, including long term residential treatment facilities and intermediate psychiatric inpatient Care. This waiver exclusively focuses on persons between the ages of 5 yrs. Old and 17 yrs. Old who have complex health and mental health needs (serious emotional disturbance). Waiver Services include: respite Care, skill building Services and family support Services. Oversight of the hcbs waiver is through new york state omh division of children and families and the nys department of health.
