Clinical/Medical Services Apicha Community Health Center CHC is a Patient-Centered Medical Home as recognized by the National Committee on Quality Assurance. A designated Federally Qualified Health Center, Apicha CHC provided comprehensive primary medical care, specialty HIV primary care, mental health services, specialty transgender health care, PrEP and PEP Services, and Family Planning, among other health care services. Common services included chronic disease management, including screening and treatment of HIV, diabetes, asthma, cholesterol abnormalities, cancer, tuberculosis, and Hepatitis initiation and maintenance of cross-gender hormone therapy and access to PrEP/PEP. Support Services included short-term mental health multilingual care management services for individuals with chronic diseases, including HIV/AIDS, diabetes, substance abuse, and psychiatric illnesses including Health Home Care Management Services care management for patients on PrEP/PEP and care management for transgender patients and care management for patients with Hepatitis B and C.
