Veteran directed care program empowers qualifying veterans to hire, employ and supervise personal care attendants to help with daily needs in support of independence. Case managers conduct assessments to determine eligibility and level of need. They review program guidelines to assist with the development and implementation of a plan of care for the veteran that best supports them in meeting their personal goals.
Congregate meal service provision, home delivered meal provision, and health promotion activities were provided to underserved members of the community for the purpose of supporting independent living. Health promotion activities include home health aid services, homemaker services, respite care, senior citizen center activities, transporation services, and nutritional education as follows: home delivered meals: 6,582 individuals with 142,022 units of service congregate meals: 10,044 individuals with 107,722 units of services skilled nursing: 145 individuals with 577.25 units of servicehealth screening: 574 individuals with 977 units of servicesenior center activity: 6,678 individuals with 44,831 units of servicehomemaking: 1,392 individuals with 9,967 units of servicerespite: 4 individuals with 36.5 units of servicetransportation: 1,274 individuals with 20,777 units of servicenutritional education: 2,683 units of service.
Personal assistance service program for medicaid eligible individuals provided so these individuals could remain in their homes instead of entering care provider residency programs serving 170 individuals with 544.5 units of service.
