Program areas at Health Quality Innovators
Hqi leads federally funded healthcare Quality improvement initiatives with disadvantaged and lower-performing providers, including more than 1,000 nursing homes and 119 hospitals advancing clinical and operational performance. We also engage population Health stakeholders in communities that have higher rates of chronic disease and fewer resources. Hqi helps hospitals in 23 states and territories decrease avoidable patient harms, such as sepsis, and has accelerated more effective infection prevention in nursing homes. Our work in communities helps to improve care coordination, decrease adverse drug events and increase the prevention and control of diabetes and hypertension for more than 3.4 million people with medicare.
Hqi is funded to: help resolve disputes between long-term care facilities and a state survey agency inspection findings; help primary care practices refer more patients to diabetes prevention and management programs; help educate providers to recognize and engage patients increase Health literacy, improve vaccination statuses, and screen them for social determinants of Health; help physician practices perform privacy and security risk assessments, optimize use of electronic Health record systems, and implement office procedural changes that advance patient safety, improve care and job satisfaction for patients and practices respectively.
Hqi leads several federal, state and local grant efforts on a variety of topics including: to assist those with un-met health-related social needs; assessing and training employees in Health care and non-health-care settings on infection prevention topics; improving chronic disease self-management by individuals with conditions such as diabetes and heart disease; investigating the use of community Health workers to support the covid-19 response and educating/engaging people with chronic disease to improve their Health; facilitating activity-based programs in the nursing home setting designed to improve resident cognitive function; and improving obstetric and post-partum maternal and infant care particularly in vulnerable populations.
