Program areas at Touchstone Mental Health
Touchstone intensive residential treatment services (irts) offers 24-hour Mental Health services in a structured supportive environment designed to improve Mental Health stability, increase personal and emotional skills, self-sufficiency, and independence. Services include education on symptom management, medication and living skills, individual counseling and group support, onsite and community activities, linkages to community services, and discharge planning for transition to community living. These three programs served 275 individuals in 2023. These programs are in bloomington, fridley, and minneapolis, with 64% successfully completing treatment and moving into a community setting at discharge.
Community Mental Health services helps people with a Mental illness improve housing stability, increase social and community connections, and improve Mental and physical Health. Services help people navigate and access medical, Mental Health, housing, vocational, financial, education, and other resources needed to reach their goals. Community Mental Health services worked alongside 1107 individuals by providing case management, care coordination, or behavioral Health home services by Touchstone connect in hennepin and dakota counties.
Community housing services helps individuals with a Mental illness find and keep their housing. Touchstone provides two housing service models to meet individual needs. The first includes housing innovation and intentional communities. These services assist people in finding and moving to a community apartment or home. Staff provide ongoing Mental Health support, housing stabilization, tenant education, and linkages to community resources that are provided in the person's home. Housing innovation served 85 people in 2023 and 85% had 12 months of stable housing. Intentional communities worked with 43 households and 98% of them had at least 12 months of stable housing in 2023.touchstone also provides supported housing at rising cedar apartments, new hope apartments, and minnehaha commons. The programs provide 24-hour onsite services tailored to individual needs and include, medication assistance, Mental Health support, independent living assistance, housekeeping, 24-hour emergency assistance, and coordination of care. 116 individuals were served by one of these three permanent supportive housing programs in 2023.
