Unity House of Troy

A child's place - provides a number of services such as a special education program for preschoolers with disabilities, child care, early intervention, and preschool multi-disciplinary evaluations for children from birth to five years, with therapeutic services in speech/language, physical and occupational therapy, psychological consultation, general skills and social work. Served 126 children in child care, and served 452 children in upk (109) and special education programs (343).

Domestic violence domestic violence services provides crisis intervention, a confidential emergency thirty-three bed shelter for adults and children, housing support services, counseling, social service advocacy and accompaniment, children's services, parenting and life skills education, information and referral, legal assistance/representation and emergency medical care. 188 adults and 154 children found refuge in thirty-three bed shelter, 1,546 individuals received non-residential services, 411 adults were provided legal and support services, 328 individuals provided transitional and long-term rental assistance and support, received 2,111 calls from the rensselaer county domestic violence hotline, 709 survivors participated in domestic violence support groups. Uhdvs staff held 51 community trainings and cooridinated response meetings.

Housing support services - Unity House offers housing support, case management, care coordination and nursing services options for adults living with mental illness, hiv/aids and/or chronic medical conditions. Services offered support individuals to live safely and self sufficiently in the community with dignity and independence. Services and programs include transitional community residences and apartment services, a situational crisis bed, supportive housing, empire state supportive housing initiative services (esshi), community housing assistance program, case management, health home care coordination, medical transportation and nursing services. Served 33 in community residences, 95 in transitional apartment services, 3 individuals in a situational crisis bed, 169 were served in omh supported housing program or housing programs for people living with hiv/aids, 43 in esshi, 56 singles and 28 families served in community housing assistance program, 151 individuals received health home care coordination, 32 were provided case management services and 327 served by nursing services.

Community resources the front door - the community resources program provides front line support for rensselaer county residents and surrounding communities, offering crisis intervention, legal and social services advocacy, emergency food, clothing and housing assistance, and holiday food baskets and gifts. 100,431 meals distributed from the food pantry, 26,229 lunches served to individuals seeking nourishment and a sense of community, 954 pre-screenings and 683 households helped with snap benefits through our nutrition outreach and education program.

Affordable housing - with the strong need for supportive housing in our community continuing to rise, Unity House now acquires and rehabilitates properties to expand the availability of quality, affordable supportive housing which helps households avoid eviction through homelessness prevention services, while also revitalizing the community.

Northeast career planning (ncp) empowers individuals with disabilities or other barriers to achieve their goals toward self-sufficiency and personal success. Ncp offers a wide range of programs including vocational evaluation, vocational training, and job placement and educational services. Our team served 844 individuals and approximately 26,598 units across eight counties in upstate new york during 2022.

Other - miscellaneous
