Program areas at FHHS
Residential Services provides opportunities for small group living in 29 community residences as well as independent living in local apartments. The goal is to provide a safe and supportive environment where each individual is encouraged to be as independent as possible. Direct service professionals provide as little or as much support as needed. Attention to medical care is paramount. Our clinical and medical employees get to know each individual, monitor all medical treatments and make sure that daily care is tailored to each individual's clinical and medical needs. Services are also provided to individuals with acquired brain injuries (abi) and the turning 22 (t22) population.
Supported home care provides case managers and nurses who support adults with disabilities living with their own family or with a home care provider. Through the adult foster care program or the shared living program, home care providers receive a tax-free stipend to support room, board and transporation costs. Fidelity House provides the home care providers with safety training, nursing supports and access to community resources.
Children and family Services provides support to families who care for a child, teen or adult with disabilities in their home. Support is offered through intensive in-home Services and a centralized family support center. These support Services include behavioral consultations, training, respite care, companionship, parent support and training. These Services have a special focus on latino medical Services, transporation and referrals. The family support center provides a full calendar of educational, therapeutic and recreational activities.
Employment training for adults with intellectual and developmental disabilities. These programs includes skills and assessment training, job placement and job retention support. The goal is to make sure each person is as successful and as independent as possible.
Day habilitation Services for developmentally disabled adults. Clinical Services such as nursing and behavorial supports as well as physical, occupational and speech therapy assessments and goal planning are provided, if needed, in combination with daily activities.
