Home Health and Hospice Care ("3hc") has provided compassionate Care to north carolinians since 1981. We strive to ensure our patients enjoy life's journey, recovery quickly and live comfortably. We believe in providing high-quality, cost effective Care that surpasses industry standards. We provide the right Care, at the right place, at the right time throughout life's journey. As part of its mission, 3hc provides Care to all patients whose condition meets the medical necessity guidelines established by medicare, regardless of the patient's ability to pay. During the year, 3hc provided approximately $197,000 in financial assistance to patients. (continued on schedule o)3hc offers the following specialty care:home Health carehome Health is a branch of medical and Care services that is focused on providing intermittent assistance to those who are confined to their homes and in need of short term acute Health Care. Although Home Care has been a staple of Health Care historically, because of the advent of modern technological advancements and resources, Home Health has expanded to include services, procedures, and therapies that in the past would be restricted to hospitals. 3hc is proud to be on the frontier of this growing opportunity to safely, efficiently, and affordably help patients remain in their communities and their homes, despite their need for consistent medical and therapeutic attention.hospice carehospice is a coordinated program of services designed to meet the physical, emotional, psychological, and spiritual needs of patients and families in their final days of life. Hospice is a special kind of caring for those nearing the end of life's journey. Our Care is centered around the concept that the final stages of life should be lived fully, with as much comfort and dignity as possible.3hc provides Hospice Care where the patient prefers, at Home, at skilled nursing facilities, assisted living facilities, and our inpatient homes (kitty askins Hospice center and secu crystal coast Hospice house). After hours careour patients are cared for 24 hours a day, seven days a week. For patients under our Care, our team offers support and direction outside of "normal working hours. "evening, night and weekend supportour Home Health Care team teaches patients and their loved ones to Care for their needs independently at Home. We understand that pain and symptoms can still worsen and major concerns may take place. When this happens, support is available, whether it is late evening, the middle of the night, or on the weekend.call 3hc firstby calling 3hc first, no matter the time or day, unnecessary emergency room visits, hospitalization, and medical bills may be avoided. Experienced, skilled nurses will be available to take your call, hear your concern and take appropriate action. Time is of the essence in these situations. For the best outcome for the patient, as well as the caregiver, our trained staff will quickly address the patient's needs and provide a plan to relieve pain, symptoms, etc. Receiving medical advice during the onset of a medical crisis minimizes the risks of medical complications, so we do our best to help in reducing the delay in care.along with the patient and caregiver, physicians can be assured that their patients will be cared for around the clock, using the same standards they are accustomed to within their practice. We strive to be an extension, working hand in hand with the patient's physicians, to provide continuity of quality care.veterans care3hc provides comprehensive Hospice services to veterans. In addition to providing all core Hospice services, 3hc provides specialized services for veterans. This includes individualized Care plans that addresses physical, medical, and psychological issues common in veterans as well as social, spiritual, and emotional support; assistance in identifying and securing military benefits and community resources; and formal recognition and appreciation for military service.3hc is devoted to improving veteran Care and understands that saying "thank you for serving our country" is not enough. 3hc strives to improve services for veterans by ensuring veterans have access to quality and compassionate end-of-life Care, recognizing each veteran and their story, recognizing and treating the unique issues facing veterans and their families, and partnering with veteran organizations.3hc is proud of its veteran population and is now a 5-star partner of the we honor veterans program. This program is a national awareness campaign that was developed by the national Hospice and palliative Care organization in collaboration with the department of veteran affairs. We honor veterans provides educational tools and resources, including promoting veteran-centric educational activities, increasing organizational capacity to serve veterans, supporting development of strategic partnerships, increasing access, and improving quality. The activities under this program reflect 3hc's commitment to ensuring veterans have access to quality end-of-life Care. Pediatric Hospice carefor a terminally ill child, what has become normal is no longer playing with friends, going to school, and begging to stay up one hour more at night. Life has become a sea of worried adults, pricking needles, and feeling tired and in pain amidst the bland walls of the hospital. While the decision for the parents to stop active treatment and begin pediatric Hospice Care is a nightmare, for the child, the focus is creating a new more comfortable environment, with less pain, less fear, and as much happiness as possible. Where there can no longer be healing or recovery, at least there can be peace and family.
