Program areas at AVOC
Presently, A Vision of Change has designed a Public Health Model of building Community HealthHubs utilizing CSU Community Health Workers and CSU student nurses as boots on the ground. Withthe additional support of the Cuyahoga County Board of Health, Fairhill Partners and thePrevention Research Center for Health Neighborhoods at Case Western Reserve University, theintent is to expand on our organizing skills, outreach/recruitment and promotion/disseminationstrategies by building a pipeline of opportunities for residents/CHWs across the spectrum ofachieving health equity and promoting health care efforts.
Recruit, train and certify 15 adults from the target population (HIV impacted) for the CommunityHealth Worker Certificate program at Cleveland State University (CSU) in conjunction with CSU(CHWs) utilizing culturally appropriate promotion materials. This includes attending and planningfor information sessions, evaluating applications and onboarding into the CHW class.
In 2019, AVOC, home of the Greater Cleveland Community Health Workers Association (GCCHWA),increased its capacity to serve as the community engagement catalyst for Cuyahoga Countyscommunity health initiatives housed under the HIP-Cuyahoga umbrella, known as REACH. AVOC notonly provides support as the connector of the clinic to community linkages strategy but the coreprograms of AVOC braided with Asset Based Community Development (ABCD) to work collaborativelywith the all functions of the initiative helping to develop and shape a comprehensive community engagement framework.

For 30 years, AVOC has provided academic empopwerment, intervention, and prevention services. Themission of AVOC is to work with a nework of grassroot organizations,cultivate relationships withresidents and provide access to healtcare to improve social disparities.
