Program areas at AVOC

Presently, A Vision of Change has designed a Public Health Model of building Community Health Hubs utilizing Community Health Workers and students in medical professions as "boots on the ground". With the generous support of our core partners and funders, AVOC's model promotes resident driven services that are positively impacting health disparities, at the neighborhoods level. Through a healthy equity lens, Community Health Hubs are aligned with Northeast Ohio's health improvement strategies (HIP-Cuyahoga).
In 2018, AVOC, home of the Greater Cleveland Community Health Workers Association (GCCHWA), increased its capacity to serve as the community engagement catalyst for Cuyahoga County's community health initiatives housed under the HIP-Cuyahoga umbrella, known as REACH. AVOC not only provides support as the connector of the clinic to community linkages strategy but the core programs of AVOC braided with Asset Based Community Development (ABCD) to work collaboratively with the all functions of the initiative helping to develop and shape a comprehensive community engagement framework.
