Program areas at Advance Community Health
Family medicine: family medicine includes comprehensive primary care services focused on prevention, acute and chronic care management. Programs include adult and senior Health care services, nutrition counseling, mental Health and substance abuse services, care management and onsite pharmacy services. Pediatrics provides comprehensive care for newborns through age 21 including pediatric and adolescent primary care, behavioral Health, and nutrition counseling. See schedule o for more information.
Dentistry: the dental program provides services for patients of all ages in a state of the art 10 operatory practice. Our services include oral hygiene, sealants, restorative services and extractions, crown and bridge work, root canals, dentures and partials. We have a specialized program that assures that children and babies are receiving appropriate dental care from the first tooth.
Pharmacy: ach operates an onsite 340b pharmacy that is available to all ach patients, including the Health care for the homeless (hch) population by utilizing grant funding to provide co-pay assistance to the uninsured or underinsured patients who may not otherwise be able to pay for essential medications. Ach also contracts with local pharmacies to provide 340b and low cost medications to patients at our other sites.
