The Aging in New York Fund empowers aging strengths and combats aging inequities in coordination with the NYC Department for the Aging. The fund partners with NYU Langone Health and DFTA to bring awareness to communities of color about clinical trial importance through Engagement and Longevity in Medicine (ELM). Additionally, through a partnership with Healthfirst, the Fund collaboratively implemented the Community Care Link model management services organization to reach out to an English-speaking list of individuals.

Program areas at Aging in New York Fund

Ccl - community care link: this model management services organization, the first of its kind, is a collaborative initiative implemented by dfta and the Aging in New York Fund, in partnership with healthfirst. The goal is to reach out to a list of english and spanish-speaking medicare recipient members identified by healthfirst, who have not connected with a primary care provider, and help them make the connection. Community navigators from 5 cbos: jasa, selfhelp, sage, bay ridge senior center, and carter burden, outreach to members residing within specified zip codes. The program was conceived before the pandemic and was revised to better meet client needs as people self-quarantined and were forced to remain in their homes. Each agency is reimbursed at set rates for the services they provide to clients, using peer place and stars to document each call.
