Program areas at Alameda Health Consortium
Eligibility & enrollment: Alameda Health Consortium coordinates eligibility and enrollment efforts with our eight member community Health centers to ensure that patients successfully enroll in medi-cal, calfresh, covered California, healthpac (Health program of Alameda county), and other public Health coverage programs for which they are eligible. Through a joint effort between the Alameda Health Consortium, the Alameda county social services agency, and our member community Health centers, our on-site medi-cal eligibility program facilitates enrollment through 1) co-location of eligibility workers at our clinics; 2) community Health center program liaisons assisting patients with new medi-cal applications; 3) bimonthly medi-cal eligibility liaison training; and 4) monitoring and coordination of our program to maximize effectiveness. For the year ending june 2023, the program completed 3,500 medi-cal enrollments, 8,100 medi-cal renewals, and 110 calfresh enrollments and renewals.
Integrated behavioral Health: ahc partners with Alameda county behavioral Health (acbh) to provide technical, administrative, and advocacy support to member community Health centers' integrated behavioral Health programs and initiatives. This includes providing support for 22 pediatric and integrated behavioral Health care coordinators, and a school-based care coordinator. For the year ending june 2023, the pediatric care coordinators spent over 168,000 minutes with their pediatric patients, alleviating the time pediatricians would have otherwise spent on care coordination and navigation.
Workforce development: ahc supports member clinics by securing third-party trainings to improve clinical practice and support targeted interventions for special patient populations such as those living with hiv, patients experiencing human trafficking or domestic violence. Ahc also facilitates partnerships with Health profession, education and training programs to support hiring of community Health workers, medical and dental assistants, and other clinic workforce to help clinics keep up with demand for services. For the year ending june 2023, ahc trained 58 community Health workers and 60 Health advocates across our eight Health centers.
