Alivia Care, Inc. was developed to provide the framework and support for the development and operation of innovative Care solutions (hospice Care, palliative Care, Care coordination, home heath, etc.) That help those with serious illness, chronic illness, end-stage illness or geriatric frailty live better and have a better experience with their healthcare needs. Alivia Care has sought to pursue this goal through collaborative partnerships and strategic investments in programs, technology, and partnerships that create the capacity to provide or support services and programs for our patients, families, and those who provide medical Care and to them. Alivia Care, Inc. has done this by providing the strategic vision to guide investment and by providing access to shared services such as business analytics, finance/accounting, healthcare/community relations, human resources, information technology, marketing and communications and quality and compliance to our patient-facing programs, which include community hospice & palliative Care, community palliative consultants, Alivia Care of Georgia (hospice of the golden isles and hospice satilla), Alivia Care solutions (Alivia supportive Care, secure Care home health and allegiant home Care) and pace partners of northeast Florida.
