Data collection - the homeless management information system (hmis) is a web-based computerized data collection system designed To capture client level information on the characteristics and service needs of people experiencing Homelessness. This data informs the region about the number of persons experiencing Homelessness, factors and patterns of housing instability, and the efficacy of the homeless response system. With this, the continuum of care (coc) can make data-driven decisions and develop strategies for preventing and ending Homelessness.
Coordinated entry is a community-wide system of nonprofit homeless service providers that connect people experiencing Homelessness and housing instability with the resources they need. This program includes subrecipient grants To partner agencies.
Continuum of care planning involves creating and implementing a community wide plan To prevent and End Homelessness and the strategic use and equitable distribution of federal funding and leveraging new resources To End Homelessness across the region. This also includes coordinating an annual funding competition for approximately $24 million in hud continuum of care program funding. This funding supports 45 housing programs for people experiencing Homelessness across Suburban Cook County.
