Three year hccn grant award for period of august 1, 2019 through july 31, 2022 to support network of 46 health centers and a three year hccn grant award for the period of august 1, 2022 through july 31, 2025 to support network of 38 health centers and a three year grant award for the period of august 1, 2022 through july 2025 to support 38 health centers working together to use health information technology (hit) to improve operations and clinical practices. Hccns help health centers leverage hit to increase participation in value-based care by (i) enhancing the patient and provider experience; (ii) advancing interoperability; (iii) using data to enhance value; two year hccn grant awarded for the period of may 1, 2021 through april 30, 2023 to support participating health centers to leverage hit and data to respond to and mitigate the spread of covid-19, and enhance health care services and infrastructure; (iv) two year ahrq grant for the period of september 1, 2021 through august 31, 2023, extended to august 31, 2024 to assess the impact of okq implementation on reproductive health metrics in the community health, primary care setting; (v) two year onc grant for the period of september 22, 2022 through september 21, 2024 to leverage fhir-based resources and the experience and capabilities of a strong partnership to enable service providers to break down barriers to integration and coordination of services in order to better address social determinants of health for individuals expereincing homelessness; (vi) two year fcc grant for the period of march 13, 2020 through october 31, 2022 to support health care providers to obtain up to twelve months of eligible telecommunications services and eligible connected devices needed to provide telehealth services in response to the coronavirus 2019 (covid-19) pandemic; hrsa grant for the period of september 1, 2021 through august 31, 2022 to strengthen the capacities of state and/or local-level organizations to respond to emerging public health issues affecting maternal and child health (mch) populations.
Foundation and corporate grants relate largely to Illinois contraception access now (ican!) Programs to improve the quality and access to contraceptive care regardless of an individuals ability to pay. Ican! Is a statewide initiative to advance reproductive health equity and to close the contraceptive care gap.
