Medical & dental Services Altamed operates 31 primary care and dental clinics in los angeles county (24 clinics) and orange county (7 clinics), in addition to two mobile units that offer medical, dental, and community outreach Services. In 2022, Altamed provided a medical home to over 245,000 patients residing primarily in historically underinvested immigrant communities in which its residents face stubborn obstacles to care and positive Health outcomes including disproportionately higher chronic disease burdens. Over 50,700 children and adults accessed oral Health Services through Altamed's 10 dental clinics, 1 mobile medical/dental unit, and 6 portable oral Health units that provided oral exams, fluoride treatments, prophylaxis, restorative Services, rehabilitative Services, extractions/other surgery procedures, sealants, and emergency Services. Independent providers contracted through Altamed's independent practice association (ipa) and practicing in Altamed catchment areas also served an additional 85,000 patients throughout los angeles and orange counties in 2022. These patients served outside of Altamed clinic walls received care and coordination through an Altamed ipa physician network that includes primary care physicians, specialists and sub-specialists--including dentistry, infectious diseases, pediatric endocrinology, and psychiatryand ancillary service providers such as skilled nursing facilities, radiology, home Health, hospice, urgent care, physical therapy, and pharmacies.
Hiv care and Services with two decades of experience as a ryan white part c provider, Altamed serves over 1,500 hiv-positive patients each year at four dedicated hiv primary care clinics in latinx communities in los angeles and orange counties. Altamed is a long-standing ryan white and minority aids initiative provider that also has longtime experience administering evidence-based, hiv prevention interventions and programming at individual-, population-, and systems-levels through multi-year cdc prevention grants and other local, state, and federal contracts.
Senior Services Altamed's 11 programs of all-inclusive care for the elderly (pace) sites allow over 4,000 nursing-home eligible frail and elderly seniors and disabled adults to continue safely living in their communities. Pace's uniquely integrative model addresses participants' complex and comprehensive medical and social needs by surrounding the pace member with a team of interdisciplinary, culturallyresponsive care providers that include physicians, nurses, pharmacists, social workers, nutritionists, therapists, and Health educators. Services include primary care/geriatric medicine, nursing care, rehabilitation therapy, family support interventions, counseling, medication management, nutrition education, social activities, and "door through door" transportation Services to participants' homes and to specialty care medical appointments. Coordinated Services ensure the member continues to enjoy the benefits and comforts of aging in place, as long as possible. The pace model embodies the philosophy of care and the core values of Altamed: patientcentered and engaging, pace is responsive to the range of whole-person needs of its patients beyond medical care that include ongoing social, emotional, spiritual, and cultural nourishment to keep them healthy, active, and out of emergency rooms and extended hospital stays.
Youth Services - Altamed provides an array of youth Services to over 1,200 at-risk teens and young adults in medically-underserved communities each year. Services include: pregnancy and std prevention programs, family planning, Health education and information workshops, linkages to clinical Services, case management for pregnant/parenting teens, parenting education, support for parenting teens to help them stay in school and life-skills development. Altamed also provides intensive tutoring for high school students, mentorship, guidance into college and technical schools, and workforce development with career pipelines in the medical field.the Altamed institute for Health equity (aihe). Through innovation and community engagement, the work of the institute is to address a wide array of Health inequities that disproportionately affect racial and ethnic groups, including addressing the multiple social determinants of Health which lead to disparities in clinical outcomes. Altamed's patient population is overwhelmingly from racial/ethnic minorities including individuals with limited english proficiency that suffer disproportionately from Health disparities. Through translational research, grants, and strategic partnerships, the institute is developing evidence-based strategies to improve patient care. The institute represents the establishment of critical resources to advise Altamed Health Services on systematically addressing key issues in Health equity by developing and advancing consensus strategies that are effective, sustainable and that can be implemented locally.altamed offers 9 specialty care Services to remove the barriers to care many were experiencing when trying to access these much needed Services. Expansion of the workforce pipelines continue to educate and train community members in ma, lvn, and nurse career pathways. Altamed has grown the newly formed civic engagement department to address social determinants of Health and train community members on how to advocate at the grassroots level for increased and particular Health access and reforms, and encourage community to get involved with local issues and vote.
