Passport services: connects families with information about the choices available in long term care. It also provides homecare services for those eligble. By arranging the most appropriate mix of homecare services, nursing facility placement can often be delayed. Passport services and assisted living service served 3,654 clients.
State of Ohio funded supportive services similar to the title iii services. These services cover ten southern Ohio counties and act as the oversight and administrative Agency contracting for the services. 768 individuals were served under national family caregiver, 97 individuals were served for veterans programs, 542 people were served by Ohio home care waiver, 804 people were served by specialized recovery services. 413 individuals were served by assisted living, 153 individuals were served by evidence-based programs, and 209 individuals were served by the hospital2home program.
A combination of programs that are grouped as federally funded supportive services to elderly including socialization, transportation, nutrition, caregiver support information & referral, chores, etc. These combined programs (oaa and arpa) served 3,957 persons. 1,732.50 days of adult day services, 3,042.75 hours of homemaker services, 1,490.50 hours of personal care, 22,737 one-way transportation trips, 10,209.75 hours of caregiver respite, 64 caregiver supplemental services and 216,861 home-delivered meals.
