Passport services: connects families with information about the choices available in long term care. It also provides homecare services for those eligble. By arranging the most appropriate mix of homecare services, nursing facility placement can often be delayed. Passport services and assisted living service served 4,255 clients.
State of Ohio funded supportive services similar to the title iii services. These services cover ten southern Ohio counties and act as the oversight and administrative Agency contracting for the services. 277 individuals were served under national family caregiver, 82 individuals were served for veterans programs, 566 people were served by Ohio home care waiver, 839 people were served by specialized recovery services. 389 individuals were served by assisted living, 55 individuals were served by evidence-based programs, and 120 individuals were served by the hospital2home program.
A combination of programs that are grouped as federally funded supportive services to elderly including socialization, transportation, nutrition, caregiver support information & referral, chores, etc. These combined programs (oaa, cares and arpa) served 3,717 persons. 1,731.50 days of adult day services, 5,407.75 hours of homemaker services, 1,615.50 hours of personal care, 17,189 one-way transportation trips, 8,526.25 hours of caregiver respite, 45 caregiver supplemental services and 230,853 home-delivered meals.
