Health center:bedford Stuyvesant Family Health Center (bsfhc) is a network of federally qualified community Health centers based in Bedford Stuyvesant brooklyn, ny. Our mission is to provide the most professional, courteous and highest quality Health care, with dignity, to those we serve, especially the underserved population, without regard for their ability to pay. In 2022, bsfhc had 66,315 patient visits. As a federally qualified Health Center, bsfhc plays a vital role in providing Health services to poor and medically underserved populations who often have difficulty accessing quality Health care.
Msa wellness program:the wellness Center program was conceived to provide support services to the patients of bedford-stuyvesant Family Health Center (bsfhc) and the wider community. The wellness Center is bsfhc response to hiv/aids in the community. Our work is centered on prevention, increased awareness to preventive services with the result to increase access to all populations who are underserved. Through our work, we have obtained funding for hiv prevention, hepatitis c navigation, case management, and community education that promotes increased access to medical, Health insurance and Health education services in the community.the year was marked by a series of impactful interventions. The approach included a wider reach into the community, increase in caseloads across all programmatic areas, development of strategic documents to establish a shared way in which the team envisioned their work, renewing relationships with community-based organizations to join linkage to care agreements and a restructuring and development of a new department structure of the wellness center.55 clients were successfully engaged in prep care. This is a huge success for the program. Our program has consistently received prep referrals externally from community partnerships and internally through our primary care, pediatrics and obgyn departments. Although there was interest in the long-acting injectable prep, only 1 client successfully received insurance coverage and actually received the injection. 95 clients engaged in intensive case management for hiv and hcv, a significant increase over the previous year. Case management was also extended to manage the agency's measures for all 354 patients with hiv to ensure their clinical indicators such as viral suppression rates are achieved. Outreach was positively impacted by increased support by case management staff, resulting in more community-based outreach, Health fairs, Health screenings and workshops being done including responding to the monkey pox (mpox) outbreak in new york city by setting up a vaccination Center to administer all 60 shots to eligible persons in the first day. These efforts resulted in 4026 people received services, Health insurance, hiv and hcv testing, linkage to care to medical services and other community-based resources.
Wic program:the Bedford Stuyvesant Family Health Center wic program is a federally and state funded program which provides appropriate healthy supplemental foods and nutrition and breastfeeding education and support for pregnant women, new mothers, infants and children under age eive. Wic has a proven track record of positive Health outcomes and befers a large number of participants for Health care and other relevant services which benefit them. Our wic program aims to improve the Health outcome of low-income women, infants and children up to 5 years who are at nutrition risk. We make this possible by providing culturally sensitive participant-centered nutrition and breastfeeding education and support as well as appropriate nutritious foods to supplement the participants' diets.we promote a healthy lifestyle with interactive encounters such as webinar and poster display. We also promote and support breastfeeding through our award winning breastfeeding team (2017 loving support awards of excellence, gold, and 2022 breastfeeding awards of excellence, gold, usda). Our monthly average participation for federal fiscal year 2022 was approximately 2,729 participants.
