Program areas at California Health Collaborative

Tobacco Programs - The San Bernardino County Tobacco Control Program works to reduce underage youth access to tobacco products and to limit secondhand and thirdhand smoke exposure in outdoor public areas. Data was collected throughout various communities in the county, with an emphasis on the Cities of Loma Linda, Grand Terrace, Victorville and Hesperia, to demonstrate the need for comprehensive tobacco prevention policies. Additionally, continued tobacco education and outreach efforts were effective in maintaining the engagement of community members and partners in all regions of the County which include the Mountain Region, Desert Region, and Valley Region.The Kings County Tobacco Control Program is conducting policy campaigns in the Cities of Avenal and Lemoore to reduce youth access to tobacco products and reduce exposure to secondhand smoke in common outdoor recreational and non-recreational areas. Data was collected in both communities to demonstrate the need for comprehensive tobacco prevention policies. The program continued to collaborate with a broad range of partners throughout the county to further the policy objectives. The FUERTE (Families United to End the Rising Tobacco Epidemic) Project is a recently funded initiative that aims to address tobacco-related health disparities among Hispanic/Latinos in the communities of La Quinta and Palm Desert. The project received funding and began recruiting staff in May 2023, which continued until early June. By June 30th, 2023, the project was fully staffed, and an office was established in the heart of La Quinta. Early partnership opportunities with College of the Desert were identified to begin volunteer recruitment.
Medi-Cal Services (CalAIM and MSSP) - MSSP continued to provide invaluable and essential services and oversight for low-come participants serving the six counties of El Dorado, Placer, Sacramento, Sutter, Yolo, and Yuba. Care Management staff have been transitioning back to completing quarterly home visits after the end of the public health emergency. CHC continues to partner as the fiscal agent for Health Net to provide health fairs for vulnerable communities in Sacramento, Placer, and El Dorado Counties through connections with MSSP and other providers. In the past year, CHC has also added CalAIM services through contracts for Enhanced Care Management and Community Supports with Anthem and the Health Plan of San Joaquin and is working on additional contracts to more comprehensively serve the Greater Sacramento Region as well as the Central Valley. CalAIM services have been largely funded by grant opportunities, have contracted with an innovative Case Management Documentation platform, and continue developing relationships and strategies to ensure program growth and sustainability.
Other ProgramsThe California Personal Responsibility Education Program (CA PREP) is a teen pregnancy and STI prevention program serving Kings County youth through comprehensive sexual health education. Additionally, CA PREP staff were able to provide reproductive health information to over 30 women who were residents in a drug and alcohol recovery home.The Community Outreach and Prevention Education (COPE) Project is an ongoing initiative that aims to provide community education on topics identified by the State of California as pressing needs. These topics include extreme heat mitigation, water conservation efforts, COVID-19 vaccination and harm reduction, and individual taxpayer identification number (ITIN) application requirements and support. The project is tasked to reach underserved communities with state provided information throughout Riverside and San Bernardino Counties.
