Advance the utilization of integrated networks for the coordination, delivery, and/or provision of health services by and among individuals, entities, agencies, and/or facilities as appropriate for population health management and enhanced quality outcomes, to develop, implement, and coordinate efforts involving the coordination of Care and the promotion of efficient utilization of local resources.in 2022, ccc provided project management support for a grant received by our lady of lourdes hospital to develop, test and distribute an online, self-guided interactive training resource tool to increase consumer knowledge and comfort in accessing virtual Care in the southern tier region of nys. The virtual online training resource was finalized in 2022 followed by an aggressive marketing campaign to individuals located in the southern tier of new york state during the months of november and december of that same year. The total impressions for that two month period reached 2,366,875 viewers.
