Program areas at Caresparc Community Connections
The sustainable health & wellness village festival (shwv) tour is a statewide multi-day community-based health and wellness festival. Shwv is an evidence-based model informed by the cdc's social determinants of health and the robert wood johnson culture of action health framework. It provides participants with ready access to healthcare and Community resources within a culturally relevant, festival-like atmosphere designed to promote meaningful engagements between communities of color and local/regional providers. Over its eight years of operation, the shwv has served over 41,000 residents in underserved and strongly impacted communities. Its innovative, evidence-based model integrates acute care services and clinical screenings with wellness education and lifestyle activities designed to address the full spectrum of determinants of overall well- being.
Caresparc Community Connections (ccc) free fresh produce market for seniors and families was inspired by the framework provided by the family caregiving advisory council (fcac) and works to improve the goals provided in their initial report. The free fresh produce market for seniors and families (ffpm) works to decrease the number of food-insecure individuals in newark and the greater newark Community. The ffpm operates in various locations from january to december.
Grandparents raising grandchildren initiative is long-standing evidence that grandparents are an underserved and often forgotten Community for raising children as primary and secondary caregivers. Grandparents are a group of people that span generations - usually comprising people forty years old and above. This means that grandparents are a highly diverse group with various complex needs. Sometimes grandparents who are primary caregivers report high levels of stress and negative consequences on their health due to neglecting their own medical needs when reassuming parenting roles (bundy-fazioli et al., 2013). Grandparents can assume parenting roles due to many circumstances with their adult children, including death, illness, mental illness, substance use, and incarceration. In the state of new jersey, approximately 41,000 grandparents are the primary caregivers of their grandchildren.
