Program areas at Catalyst for Change
Development of our new technology hub for the chw. This platform will be used for our community health workers, so that they can do their jobs more efficiently. The portal includes scheduling, a calendar, notes, coordinating care teams around clients and a custom reporting tool that will be used to extract data more efficiently than a person could manually enter the data. The custom reporting tool is significant as it easily allows us to extract data and report to donors. The platform is significant for chw's as they will no longer have to navigate multiple online platforms piecemeal. Allowing chw's to conduct all of their direct client services in one place, in the virtual world. Chw training curriculum; this curriculum is specifically built and designed to train Catalyst for Change chw's. The curriculum has a strong focus on serving rural populations specific to behavioral health and through the use of technology. The curriculum trains chw's on how best to provide preventative crisis and care. Provider network; our provider network consists of dozens of therapists and psychiatrists who provide a variety of local and or telehealth services. Our chw's are the link to connecting our clients to the provider network. Navigating the provider network requires a live variety of skills. Which include but is not limited to; knowing which telehealth platforms to use for each provider, assisting clients with intake paperwork, providing the necessary technology required for telehealth visits, and being sensitive to each provider's schedule and availability.
