Program areas at CCHP
The extended care program is a medicated assisted treatment program that is designed to meet the individual needs of those struggling with opioid addiction. The goals of the program are to help patients maintain abstinence from illicit drug use while addressing their medical, mental Health, family, interpersonal, housing, legal, and vocational needs. Treatment is received under the supervision of a provider and is tailored with active patient participation to ensure the best opportunity for recovery.during 2022, 476 patients were served with 8,090 visits. In 2021, cchp merged two other medication assisted treatment programs into cchp's extended care program, which increased our total number of unique patients and visits within this program.
At cchp, primary care Health services extends beyond a once a year physical with your primary care provider. Our team of physicians, care managers, patient navigators and counselors and nurses work together to address the medical, social, and emotional and child development needs of families. Cchp covers the spectrum from pediatrics to geriatrics and includes prevention and/or treatment programs for hiv, hepatitis c, hypertension, diabetes, and asthma. Our emphasis on Health education and early treatment helps our staff intervene before minor issues become life-threatening.our playrooms provide a space for early childhood development services, as well as enabling parents to have a safe place for their children during their appointments. During 2022, 2,215 patients were served with 8,034 visits.
The family and employment services program helps individuals and families overcome substance use issues, stabilize family relationships, expand employment options, and improve overall Health and mental Health status. Treatment is tailored with active patient participation to ensure the best opportunity for recovery. During 2022, 270 patients were served with 3,941 visits.
