Program areas at Champlain Community Services

Champlain Community Services offers Community supports that enhance consumers' access to activities in the local Community, promote independence, and enable participation in a broad range of experiences. Through this program, consumers develop independent living skills, establish and enhance friendships, volunteer in the Community, pursue recreation and leisure activities, and get healthy through exercise and fitness programs.champlain Community Services (ccs) supported employment program way2work provides a unique continuum of employment supports: from pre-vocational experiences for current high school students, to school-to-employment supports for graduating or transitioning students, to adult career development supports from first job through retirement. Ccs also offers self-employment supports, helping entrepreneurs with disabilities launch micro-businesses. In fy20, way2work made 10 new job placements, with 78% of ccs consumers employed. Way2work participants earned more than $200,000 in collective wages and generated an ssi savings of nearly $93,000.bridging at ccs helps high school-aged youth with disabilities step confidently into their adult years by introducing them to meaningful Community opportunities, exploring topics connected to healthy adult living and building healthy relationships. In fy20, 15 high school students from five area high schools completed the year-long bridging program at ccs, gaining valuable career exploration, Community engagement, independent living, and self-advocacy experiences.
Ccs offers a broad array of supports to people with intellectual disabilities and autism: at work, at home, at school, for life. Our personal approach ensures all Services are personalized, coordinated, and delivered in the Community. This year, nearly 100 ccs consumers received high-quality supports from our dedicated staff of direct support professionals (dsp), based on a detailed individual service agreement (isa), carefully managed and integrated by a team of professional service coordinators. Dozens of partnerships with area businesses, organizations, and neighbors make sure these rich experiences occur within and throughout our local Community
Champlain Community Services offers home and shared living supports ranging from strengthening family-based living options to offering a few scheduled hours per week in a consumer's home helping them develop life skills (such as cooking, housekeeping, and personal finance) to 24/7 contracted support in a shared living provider's home. This program focuses on ensuring the consumer's health and safety, maximizing independence, addressing individual preferences and needs, creating a supportive and inclusive home environment, and enhancing quality of life.
