Choices Healthcare provides personalized care and comfort to help patients and families live better with serious illness through end of life. During the current year, Choices Healthcare provided over 165,000 patient days of hospice care to patients and their families and served 3,432 patients during the year, regardless of the patient's ability to pay. Four percent of the patient days of hospice care were charitable care. Providers completed almost 3,300 hospice and palliative care visits to patients in their private homes, nursing homes, hospitals and hospice inpatient center. The bereavement program supported 8,450 individuals during the current year through 230 support groups, almost 3,400 telephone calls, and 671 individual visits.
