Christiana Care Health Initiatives

Christiana Care Health Initiatives provides charitable primary Care, home-based primary Care to homebound patients, cardiology, substance abuse and other healthcare-related services to the community in support of the Christiana Care Health system. Primary Care services were provided to the salem county community in new jersey. Providers within two primary Care practice locations performed a total of 16,246 in-person office and virtual visits for fiscal 2023. Christiana Care Health Initiatives provides outpatient healthcare services to the community in support of the Christiana Care Health system. Home-based primary Care services were provided to homebound patients in the new castle county and middletown communities in Delaware. Providers including both physicians and nurse practitioners performed a total of 6,060 home visits for fiscal 2023. Christiana Care Health Initiatives provides outpatient healthcare services to the community in support of the Christiana Care Health system. Behavioral Health services for treatment of substance use were also provided to the community during fiscal 2023 in furtherance of the organization's primary tax-exempt purpose and the Christiana Care Health system. Providers including psychiatrists and counselors performed a total of 3,597 visits for fiscal 2023.
