Program areas at Columbia River Health
The purpose of Columbia River Health (crh) is to protect and improve the quality of life for all people, particularly the medically underserved and vulnerable population, by providing access to high quality primary and preventative healthcare. In addition, crh offers a sliding fee discount program for those below 200% of the federal poverty guidelines. Eligibility is based on income and family size. Applicants who qualify have their charges written off and are only required to pay their applicable sliding fee. Crh had an overall increase in patient visits from 11,198 in 2022 to 11,551 in 2023. In addition, we also offer a dental program for our sliding fee eligible patients who have no dental insurance. This work is done in conjunction with advantage dental clinics for emergency and preventative treatments. We offer in-house limited x-ray and have 3 staff members who are x-ray certified. The majority of our patient-facing staff is spanish speaking. In addition, a language line is also available for any non-english speaking patients. The language line service is used anywhere from 15-180 minutes per month. This is a great resource especially for patients who do not speak english or spanish so that we can ensure we meet everyone's needs. We offer integrated behavioral Health to support our patients. Our providers also offer substance use disorder treatment. The pharmacy is located in the local grocery store in town, which makes it a convenient location. Additionally, crh is contracted with capture rx, who contracts with the local hospital and safeway pharmacies in the region to dispense 340b drugs. All of our patients are eligible. A computer program scrubs the prescriptions, and gives them the better pricing of the pharmacist stock and the 340b pricing. The meds are dispensed by the contracted pharmacies, and restocked by capture rx. They bill us for the meds restocked and also issue us payments from proceeds from the program. We are an out stationed outreach worker (osow) with the state of Oregon. We provide assistance with the Oregon Health plan application process (medicaid). Free transportation is provided for our service area patients. Spanish-english bilingual staff is throughout the various departments, to assist with our high number of spanish speaking only patients. In 2023, the uninsured patients were 9% of the total, medicare and medicaid was 49% of the total. On average, 2% of primary care visits were via telehealth and 4% of behavioral Health visits were via telehealth for 2023.
