Program areas at Community Health Hub
The Community Health Hub (chh) was awarded a school based Health center capital development grant through Health resources and services administration (hrsa). The grant provided $500,000 over a 2-year period to construct and equip functional school based Health centers (sbhcs) within the three school buildings of lakeland school district; lakeland high school, lakeland elementary school and mayfield elementary school. In implementing the school based Health center capital project, chh has partnered with the lakeland school district to achieve improvements in students' access to Health care at the three schools in the district by renovating existing nurses' stations into the school based Health centers and upgrading equipment needed for primary care delivery.over the 2-year grant period, chh has worked with a local architect and contractor to design and renovate the existing nurses suites. The redesign includes 2 functional exam rooms, observation areas, a waiting area and nurses work area at each location. Chh also purchased new clinical, information technology and fitness equipment to be used at each of the sbhc locations. Clinical equipment included items such as stethoscopes, ekg machines and exam tables. Information technology equipment included networking infrastructure, laptops and fax/copy/scanners. Fitness equipment included treadmills, elliptical and rowing machines. State-of-the-art kinesthetic pedal desks were also purchased for the district. The entire project was centered around the long-term goal of improving the delivery of Health services and the Health of the lakeland school district student population.the long-term intention is for the chh to operate the new sbhcs utilizing a cost-effective and physician led patient centered medical model of care in full collaboration with the current medical staff employed by the schools and the Community physicians caring for the students. The operational sbhcs within lakeland school district will be dependent on future funding opportunities for operations and projected sustainability revenues.
