Individual Service support-maintains contact with people in disability related programs of the Illinois department of human services to determine if services are appropriate, to monitor individuals progress, and to ensure that consumers rights, health, interest and welfare are sustained.
Case coordination-gather information including medical, psychological, educational, and social background, and make Service arrangements for individuals who require assistance. The skilled advocates at cso assist in the development of individual person centered plans for Community based human services and supports, and work with each individual and/or their family/guardian to identify goals and stragegies that address uniquie needs and maximize their potential. Cso plans and arranges services, such as medical care Options, employment, accessible transportation, and recreational activities.
Dors traumatic brain injury
Pre-admission screening-coordinate eligibility determination, individual assessments, Service applications, and Service arrangements for people with developmental disabilities. Cso serves as a clearing-house for persons with disabilities, and/or family/guardian, who are seeking services. It compiles and disseminates information pertaining to disability issus, resources, services, and events; and assertively educates federal, state, and local policy makers about disability issues.
