Program areas at Community Uplift Program / Cody Severson MBR
Community connect: care coordinators help participants establish goals and connect them with Community resources related to their needs, such as recovery, housing, employment, and criminal pasts.
Pass-through funding rent help: through a contract with nd rent help, as applicants are approved for funding, they could have past owed rent from previous landlords paid for through the nd rent help system. The funding would come from nd rent help to Community Uplift Program, and then Community Uplift Program would pay to owed landlord.
Nd housing transitional: individuals are assigned to a Community member as their applications were approved for payment in the nd rent help system. They assist Community members with keeping up and recertifying their status through the life of the Program, understanding their leases, and generally succeeding in their rental agreement.
We currently offer care coordination and certified peer support services through free through recovery, 1915i care coordination, and 1915c service management.
