Medicaid waiver programs - passport and assisted living:passport is a community-based long-term care program for medicaid eligible older adults 60+ that provides in-home services instead of permanent nursing facility placement. Services provided may include: intake, in-home assessments, care management, personal care, homemaker, adult day services, emergency response units, Home delivered meals, transportation, Home modifications, chore and specialty consultations for nutrition and social work counseling.assisted living is a community-based long-term care option for medicaid eligible older adults that provides an alternative to nursing facility placement by bridging the gap between in-home services and permanent nursing facility placement. Residents have access to assistance 24 hours a day, one time community transitions services, if transitioning from a nursing facility, as well as care management.passport and assisted living have level of care and financial eligibility requirements that must be met in order to qualify for program services. Approximate number of unduplicated members served 2,289.
Care coordination program:the care coordination program provides services to older adults who want to remain independent and reside in the community, but do not meet financial eligibility for the medicaid waiver programs. Services include care management, personal care, homemaker, adult day services, emergency response units, Home delivered meals, and family caregiver services and supports. Approximate number of unduplicated clients served 511.
Mycare Ohio is Ohio's dual demonstration pilot with cms and the state of Ohio to provide long term services and supports to medicaid and medicare eligible adults over the age of 18. Services provided to eligible adults include care management, care plan and service plan review, and regular face-to-face visits.approximate number of unduplicated members served 5,677.
Elder rights program - ombudsman:trained and state-certified ombudsman advocates promote quality compassionate care for consumers of long-term care services through complaint investigation, information, education and advocacy. Ombudsmen protect the rights of all consumers of care, regardless of age and work to resolve their care and service issues. General information and advocacy are provided to consumers and families on topics such as selection of ltc providers, facility performance and guidance on rights issues. Advocacy is conducted by unannounced on-site visits as well as through proactive telephone contacts to residents and families.ombudsman staff and volunteers provided 15,012 hours of service in the thirteen county region. In carrying-out their work, ombudsman representatives provided core services to 7,683 consumers and families.
Specialized recovery services (srs) & Ohio Home care (ohc) - the specialized recovery services program is a medicaid program for adults 21 years of age and olderwith serious and persistent mental illness, end stage renal disease, and/or a diagnosed chronic condition.the Ohio Home care program is a medicaid program for individuals under the age of 60 diagnosed with chronic conditions. Although these individuals may have access to basic health services through medicare or private insurance, medicare and private insurance do not pay for service coordination and community support activities covered by medicaid.approximate number of unduplicated clients served 364.
Grants and allocations to providers and other community programs include information and assistance, legal services, transportation, nutrition education and consultations, health and wellness programming, community living specialists, caregiver support, and minor Home modification. Efforts to maximize community involvement in isolation prevention, increase digital connectivity, and provide critical resources and supports directly related to health and well-being. These grants and funds provided additional education and activities for older adults most vulnerable to pandemic-related stressors and dangers through initiatives like: Aging is an art form, Direction Home dialers, and the wellness connection activity books.
