Heartland Services strives to provide quality services that promote personal choice, self-respect, and dignity. Our offerings include Medicaid waiver residential, day, and personal assistance within four counties. We are proud to serve thirty eight individuals through both group homes and supported living. These services enable the individuals we support to achieve greater independence in daily living skills and increased community involvement.

Program areas at Heartland Services

Medicaid waiver residential, day, and personal assistance. Residential - this includes both group homes and supporting living in four counties. Thirty eight individuals are served. Achievements are related to community activities and daily living skills. Day - this includes Services to individuals in prevocational training centers, community based activities, opportunities for work, and serves approximately one hundred and fifteen individuals. Acheivements are related to providing oversight/trasportation for physicians/appointments and providing leisure and learning activities. Personal assistance - provides assistance to individuals who live in their own home. Acheivemnts include helping individuals with daily life skills.
