As part of our ongoing commitment to address the Social Determinants of Health (SDoH), we have initiated screening and intervention for non-medical needs that our patients often encounter. This endeavor is a collaborative effort, with us working closely with esteemed partner organizations such as Valley Health Systems, Valley Assistance Network, and Healthy Families, among others, to provide comprehensive resources and support. As of 6/30/2023, the clinic community engagement specialists have helped over 1200 patients with resources regarding food, housing, and employment opportunities. They have also helped those who might qualify for Medicaid services with their applications and filled out the applications for financial assistance at our local health system, Valley Health, for specialty referrals.
