Program areas at Eastmont Towers Community

Eastmont is a full continuum provider. Residents, who choose Eastmont as their home, enter into a life care agreement, which provides access to different levels of service on one campus. The journey begins in independent living. There are 121 apartments of various sizes and design, which are located in three different buildings on the property. The apartments are kept up to date and renovated with quality construction. Living independently at Eastmont includes services such as a meal plan, transportation to shopping and medical appointments, bi-weekly housekeeping, maintenance, and a rich program and events schedule. Eastmont provides 24-hour emergency response to apartments and residents have access monday - friday to a nurse in one of two clinics on campus. Eastmont also provides many opportunities for spiritual fulfillment including a chaplain who conducts services on campus and will assist with spiritual needs when requested. The health care team will help residents navigate different levels of care if their health needs change.
The next level of care at Eastmont is assisted living for those individuals who may need assistance with activities of daily living. The seasons is licensed as assisted living and is located on the Eastmont campus. The seasons provides 41 one-bedroom apartments and uses an activity-driven social model with an emphasis on fellowship, enrichment, and educational opportunities. Residents have access to licensed professional nursing staff on campus 24 hours per day. Individuals have tailored programs that can include assistance with activities of daily living, medication management, health maintenance activities, transfers or ambulation, oxygen, and additional companion or home health needs.
Eastmont also provides a health center, which is licensed for medicare and provides both short-term rehabilitation in 10 rooms and long-term care in 11 rooms. They are designated as semi-private (can accommodate two occupants), but in response to the market, Eastmont operates them as private rooms. The health center provides skilled nursing, and physical, occupational, and speech therapy in an interdisciplinary environment. Additionally, spiritual guidance, social work, case management, and care services are provided for the overall psychosocial well-being of the patients. The care center is guided by a medical director, licensed nursing home administrator, and director of nursing.
The monarch is a free-standing six-bed facility, licensed as assisted living, dedicated to enhance the continuity and quality of hospice care delivered to terminally ill people in the Community. The monarch provides a beautiful home-like environment with space for family and friends to gather. Trained staff partner with all area hospice agencies to provide comfort/palliative care to residents and their families. Residents must be established with a hospice care provider and need partial or total assistance with the activities of daily living. Medical supervision is arranged for by the resident, family, or other persons before admission.
