PRE-EXPOSURE PROPHYLAXIS (PrEP) PROGRAM STIGMA REDUCTION ACTIVITIES SERVICE NAVIGATION Empowering Transgender services identified 10 transgender clients with positive test results who were referred and linked to care within 24 hours. The clients were referred to the following location for follow-up care and case management. Clients linked to Eastern Virginia Medical School Infectious Disease Department for medication management and enrollment into Ryan White Part A/B. Clients are linked to navigation and case management services via the LGBT Life Center, Virginia. Clients linked to the Pharmacy Shoppe for Medication prescription refills. Although housing is a challenge in our area for stabilization, clients were referred to charitable arms for housing resources for community stability. Increased HIV testing among priority populations. Increased HIV diagnoses among priority population clients unaware of their HIVs Status. Increase access to prevention and essential support services. Increase receipt of integrated screenings. Increase access to condoms by priority population. Increased screenings, referrals, and linkage to PrEP. Increased Linkage to or re-engagement in HIV care among persons with previously. diagnosed HIV, not in care, within 30 days. Increased access to Partner Services among persons newly diagnosed with HIV or qualifying STI. Increased access to appropriate medication adherence services.
