Program areas at FCS
Medical case management:family and Community Service of Delaware County, Inc. (fcsdc) provides medical case management (mcm) for Delaware County residents who are living with hiv. Medical case managers conduct ongoing assessment of needs and facilitate linkage to medical care, medication access, and other resources. These services support treatment adherence and retention in medical care. In fy2023, 128 individuals received mcm services.
Behavioral health services: fcsdc provides outpatient mental health counseling for adults, adolescents, and children. Psychiatric services are available for program participants including psychiatric assessment, medication prescription if needed, and medication management. During fy2023, 289 individuals received counseling services.
Housing services:in addition to the new Family transitional housing program, fcsdc operates a transitional housing / rapid rehousing program for adult men experiencing homelessness. Program participants are housed at our ralph moses house (rmh) facility in chester, pa for up to three months. During that time they work with a case manager to obtain affordable housing. During the rapid rehousing portion of the programs, a rental subsidy is provided for up to 24 months while participants build financial independence. During fy2023, 14 men were housed at rmh and 24 men received assistance through the rapid rehousing program.fcsdc also manages permanent supportive housing for families and chronically-homeless individuals. The program provides rental subsidy and case management services for up to 10 families and 15 individuals at a time. During fy2023, 10 families (consisting of 27 individuals) and 17 chronically-homeless individuals were supported through this program.
Emergency food distribution:fcsdc operates a mobile food pantry that delivers food boxes to low-income residents of Delaware County who have limited access to food resources outside their homes due to disability, illness, or age. Referrals were accepted from medical providers, social workers, case managers, and other Service providers throughout the County. The amounts and types of food delivered were similar to what a household would receive at a food pantry if they had access. The mobile food pantry also delivers food boxes to homeless individuals and families temporarily housed in area motels. In fy 2023, the mobile pantry made 434 deliveries serving 935 individuals (495 adults, 336 children, and 104 seniors).
