Hong Fook Adult Day Health Care and Hong Fook Center Harrison Street Hong Fook Community-Based Adult Services (CBAS) serve a vulnerable population of frail elderly and adults with chronic medical or mental health conditions, cognitive impairment, and/or disabilities. The mission of Hong Fook Center is to promote the optimal physical, emotional, and social well-being of our Participants and their caregivers; to minimize hospitalizations, and to delay or prevent nursing home placement. Center provides individualized, comprehensive, and culturally sensitive services as follows: Professional nursing care and health education Chronic disease management and prevention Mental health services Medical social services and case management Recreational, therapeutic, cognitively stimulating, and cultural activities Physical therapy & occupational therapy and speech therapy services Registered dietitian services Personal care, transportation, and nutritious meals Training, support, respite, and counseling to Participants' families and caregivers Since the full reopening of in-center congregate services in October 2022, Hong Fook Center multidisciplinary team has worked tirelessly to help our participants to adjust to the reopening. In addition, Hong Fook Center has incorporated a new service known as Emergency Remote Service (ERS) to all participants who are facing specified emergency situations; and are unable to attend the center. Our in-center services and ERS ensure the continuity of care of our participants. Hong Fook Centers adopt a Person-Centered model of care which emphasizes meeting the individuals unique needs. Participants and their families engage in the care planning process. As an essential part of the Home and Community Based Services, Hong Fook Center continues to collaborate with a wide range of community, health care agencies to meet the changing needs of our participants and caregivers, Hong Fook Adult Day Health Care serves African-American, Korean and Chinese participants. The center offers morning and afternoon sessions each day.
Community Health Home (The Housing and Community Services Department) houses two essential programs: the Oak Street Community Cabins (OCC) and the Community Ambassador program. Oak Street Community Cabins (OCC) restores hope and supports houseless individuals in Oakland Chinatown and the surrounding area to secure and maintain stable housing. Within our 24/7 emergency shelter, we continue to address basic needs for up to 38 individuals, offering them respite from the daunting challenges of street life and enabling them to focus on their journey toward stable housing. In the past year, we have successfully provided emergency housing for a total of 63 women/women-identifying individuals, ranging in age from 20 to 65, provided over 8,300 meals, and housed 16 participants. Our Community Ambassador program continues to be committed to building community, reducing violence, and restoring a sense of security for all community members, including residents, workers, merchants, and visitors. In the past year, we have defused 91 tense situations, provided 6,098 outreach and intervention services to houseless individuals, and offered guidance and referrals to community resources 2,697 times. Additionally, 144,690 pounds of trash were removed, 484 blocks were power washed, and an estimated 19,375 square feet of graffiti were eliminated.
