Family Connects International (FCI)

Supported nurse-home visiting programs for famiies of newborns.fci supported 35 implementing community partners in 14 states, including four new partners that were onboarded during this fiscal year. Their catchment represents 1.6% of all births in the united states. Across all supported sites, 15,713 home visits were completed.fci trained 99 registered nurse home visitors. Nurse training includes education in therapeutic communication, cultural humility and implicit bias, trauma-informed care, supporting all Family structures, social determinants of health, the Family Connects model, postpartum and infant health assessment, infant feeding, the parent-child relationship, safe sleep, intimate partner violence, perinatal mood disorders, management of infant crying, and substance use disorders.fci began offering monthly partner and network data snapshots. The partner snapshots offer each implementing partner program outcome and de-identified aggregate patient demographic data, which are reviewed by their implementation specialist and used to support continuous quality improvement and health equity.
