Program areas at Family Service of Chester County

Project one:project one provides medical case management services, support groups, and mental health counseling to adults with hiv/aids in Chester County. Our primary work revolves around medical adherence and addressing client barriers to consistent medical care. Case managers and clients work together to achieve the ultimate goals of empowerment, optimal health and independence.project one is confronting the growing number of aids cases through an evidenced-based prevention program that focuses on condom use, condom negotiation, and safe sex education. Our goal is to increase awareness of hiv/aids in the community, teach risk reduction techniques, and ultimately reduce the incidence of hiv infection and transmission.results that make a difference: 85% of project one case management clients regularly follow-up with their medical appointments. 82% of project one case management clients who receive counseling services report feeling better and healthier.
Supportive housing for transition age youth:new to the Family Service continuum is our supportive housing program which provides case management and coordination of financial assistance to transition aged youth (ages 18-23) who were in placement with Chester County department of children, youth and families (cyf) or the juvenile probation office (jpo).these youth are referred to the program by Chester County's housing action team (hat). Case managers provide intensive housing support services for youth, meeting on at least weekly basis until nearing successful completion of the program. Results that make a difference:95% of the youth who were discharged exited the program into safe and affordable housing.95% of youth who secure housing,increased their portion of rent and utilities within six months of receiving assistance.
Counseling program:the counseling program offers a supportive, nurturing and confidential environment for families and individuals to learn how to deal with common problems of every day living. Family Service's licensed clinical social workers help clients cope with the emotional confusion and pain associated with strained relationships, the trauma of separation or divorce, the devastating effects of post partum depression, loss of a loved one, parent-child conflicts and other issues. Both individual and group counseling are offered. Results that make a difference:90% of counseling clients who attend 4 or more sessions increased functioning level. 88% of counseling clients who attend 4 or more sessions met some or all goals on their treatment plan.
