Healthy families: a voluntary, home visitation program offering intensive support to parents and their children from birth to age 5 in an effort to prevent child abuse and neglect. Brown & door counties. 480 welcome-baby assessments and 1,240 home visits conducted. 101 families enrolled, 109 children served. Early head start: a federally funded program that works with low-income infants, toddlers, pregnant women, and their families. Services are managed in collaboration with cesa 7, and focus on enhancing children's physical, social, emotional, and intellectual development. Brown county. 50 families served.
Residential treatment: provides intensive Services to youth who are unable to remain in their homes or other community-based settings due to significant emotional and behavioral problems. Northeast Wisconsin. 39 youth served.
998 Wisconsin lifeline: a national suicide prevention lifeline call center serving all of Wisconsin. Provides short-term crisis counseling over the phone for people of any age, 24 hours a day, 7 days a week, and free of charge. 25,134 telephone crisis calls answered. 9,670 families served. 4,023 follow-up contacts provided.
Parent connection: an early learning, home visitation program for first-time parents focusing on child development, health issues, Family support, and educational opportunities. All Services are provided in an effort to prevent child abuse and neglect. Outagamie, winnebago, shawano, and parts of calumet counties. 121 families enrolled in home visitation. 1,785 home visits conducted.
The counseling clinic: provides comprehensive counseling Services for individuals of any age, families, and/or groups on a variety of relationship and mental health issues. Green bay & fox valley areas. 1,385 individuals served.
Crisis center: provides short-term crisis counseling in person or over the phone for people of any age, 24 hours a day and free of charge. Brown and 12 northern Wisconsin counties. 4,516 in-person counseling sessions; 19,349 telephone crisis calls answered.
Sexual assault center: provides free, confidential advocacy and support Services to sexual assault victims and their families, 24 hours a day. Also focuses on community awareness, education and prevention. Brown, door, marinette & oconto counties. 1412 primary and secondary victims assisted. 8,400 people reached through prevention education presentations (through sac and willow tree presentations).
Day treatment for children & adolescents: provides intensive counseling Services to youth who are exhibiting severe behavioral or emotional problems. Brown county. 88 youth served.
Our place: an 18-bed community-based residential facility for adults who have been diagnosed with a serious mental illness and are unable to care for themselves. Green bay. 20 individuals served.
Self program: addresses the needs of youth who demonstrate significant personal or Family issues with a goal of preventing out-of-home placements and improving Family function. Brown, calumet, door, outagamie, and shawano counties. 134 families served. 4,922 home visits conducted.
Silvercrest group home: a group home for teenage boys, Services are designed to stabilize delinquent behaviors and improve Family relationships. Winnebago county.
Women's recovery journey: women's recovery journey is an outpatient treatment program in brown county for women who are struggling with addiction and/or other mental health issues, and their families. Our goal is to help women recover from their addiction so they can better care for themselves and their families. 48 women served.
Willow tree cornerstone child advocacy center: provides Services to children who are suspected victims of abuse, neglect, or witnesses of crime in a non-threatening, compassionate, and child-friendly environment. The center brings together a team of specially trained professionals who evaluate and investigate cases of child abuse, while attending to the child's immediate psychological and medical needs. Therapy and support Services are also provided. 572 children served. 450 child forensic interviews conducted. 12,684 children and adults served through prevention education.
Families first: works with brown county human Services to offer prevention and intervention Services with a primary goal of reuniting and preserving and families. Brown county.
Windows to work: a voluntary program designed to assist inmates and parolees of the taycheedah correctional institution and the kettle moraine correctional institution in making successful transition to the life outside of prison. Brown, manitowoc, sheboygan, oconto, kewaunee & door counties. 82 individuals served.
Pathways to employment: provides youth, ages 14-24, with intensive case management Services focused on setting and attaining both educational and employment goals. Brown county. 107 individuals served.
Triumph: provides integrated mental health, case management, and educational Services to youth experiencing a mental health crisis that is impacting their educational success. Students come from one of our three partner schools. Services include individual assessment and treatment, group counseling, education, and case management. Funding comes from the community foundation of the fox valley and third party billing. 78 youth served.
Beacon: the beacon program is a collaboration between howard-suamico, pulaski, west de pere school districts and Family Services of Northeast Wisconsin to bridge the gap between school and mental health treatment and Services. 36 families served.
Community connections: provides comprehensive voluntary Services to families that are reported to, but not served by, the formal child protective Services system. Services include case management, home visits, collaborative goal setting between the ccp worker and the primary care giver who may need support with housing, mental health, parenting classes, child development, respite, Services for children with unique needs and meeting basic needs such as food, clothing, diapers etc. Provided in partnership with outagamie county department of health and human Services, children, youth, and families. 709 individuals served .
Day report center: the day report center and its supportive Services are designated to serve as an alternative to incarceration for non-violent adult offenders with Services including assessment and case management, day reporting and monitoring, and skill building. Winnebago county.
Student assistance program: provides a variety of school-related counseling and intervention functions in local schools. Brown county. 115 students served.
