Description
Family Support Network (FSN) provides services and resources that help children with special needs reach their full potential. FSN seeks to improve the lives of children with special needs. Through multi-tiered assistance efforts directed towards their families, children with developmental, emotional, cognitive or complex health conditions are guided to services and resources that enable them to achieve their maximum potential. FSN believes that every child should have the chance to grow up happy and healthy.

Program areas at FSN

Parent mentor services program - provides guidance and Support to parents receiving Family reunification services through the dependency court system. Assisted approximately 214 families reunified and 310 resources/ referrals provided to families.
Reach: resources, education, advocacy, and help - as of this year, fsn became a vendor of the orange county regional center. The service coordinators refer cases to our fsn reach parent mentors asking for assistance with helping clients apply for benefits such as ssi, ihss, medi-cal etc. We also assist with general resources such as helping them apply for calfresh, finding low-income housing, iep assistance, behavioral health, and connecting them to services such as ot/pt, and speech. The total number of cases was 269 and resources given was 1,064.
Wraparound - provides community based services to help children who are placed or at risk of placement in a group home, live in Family settings. Screened 310 individuals for qualifications for parent partners to work with orange county wraparound program and provided over 6,867 items to wraparound staff to be provided to individuals in need. Completed 620 fidelity and satisfaction surveys.
Parent-to-parent outreach - when a patient-caregiver is affected by a child with special needs (medical, intellectual, developmental and/or behavioral), they are partnered through the parent-to-parent outreach program with a parent mentor, who as parents of children with special needs can offer one-on-one Support, general suggestions, and information immediately. Total families served was 36, the total number of resources given was 162.voice options - distributed 72 units to qualified individuals.emergency needs program - the emergency needs program provides basic needs items, such as diapers, food, clothing, baby formula, and cribs to families who have children with special needs or children "at-risk". Last year we distributed 197 items related to emergency needs.bloom - free behavior management program for families with children ages 0-5 who are experiencing behavioral issues due to times of transition, sudden or unexpected changes in routine (for example covid), or existing diagnosis. Bloom received 242 referrals from the developmental screenings for a total of 360 families and 1,528 individual sessions (max 5 each family).developmental screening program - free, comprehensive screening for children ages 0-5. Screened 562 children for developmental milestones. Follow-up and assistance provided to those receiving a referral for additional services, 723 services provided.
