Farm Bureau Group Welfare Trust

The trust provides healthcare benefits for substantially all active employees of fbl Financial Group, Inc. and certain affiliates. The plan provides health benefits ( medical, hospital, surgical, drug, and major medical to all eligible employees of the participating companies regular employees who work at least 20 hours, on average, per week) and to their covered dependents. The plan also provides continuation of certain benefits upon termination of employment through cobra.
