Program areas at FSL Pathways

Through its affiliates, foundation for senior living (FSL) has been making Arizona a safe place to age since 1974. FSL assists over 42,000 arizonans annually through more than 25 programs centered around four primary areas: safe & affordable housing, nutritious meals, health & wellness, and caregiver support. In the fiscal year 2022-23, FSL continued its commitment to providing secure housing by offering shelter to over 1,500 arizonans each night. This encompassed various housing options, including single- and multi-family homes, affordable housing complexes, and residential behavioral health group homes. FSL also actively trained and monitored sponsors involved in adult foster care and adult behavioral health therapeutic home programs. (continued on schedule o) as the largest provider of weatherization services in Arizona, FSL's weatherization and home improvement initiatives cut energy bills and improved health for over 8,000 homeowners. In the area of nutrition, FSL provided 200,000 meals to families in need via food pantries, congregate and home-delivered meal programs. Health and wellness initiatives provided transitional care for over 900 patients upon hospital discharge and operated adult day health facilities, delivering over 150,000 hours of respite care to family caregivers. Throughout the fiscal year 2022-23, FSL remained steadfast in its mission, making significant strides in ensuring the safety, health, and well-being of arizonans of all ages.fsl Pathways assisted group living program operates 17 community-based homes throughout maricopa county, designed for adults, ages 18 years and older, with special medical and psychiatric needs. These homes are state-licensed as residential behavioral health facilities. Trained staff provide continuous supervision and support to promote self-reliance and independence in the least restrictive environment possible, avoiding institutionalization for these clients. In fye 2023, 28,954 bed days were provided to 122 individuals, and seven clients graduated to a lower level of care. The program provides room and board, all meals and snacks, hygiene items, transportation, accompaniment to and advocacy in medical appointments, group and individual therapy.
FSL Pathways adult behavioral health therapeutic home program is a network of foster families providing in-home support services for behavioral health residents to assist them in achieving their service plan goals and objectives. In fye 2023, 7,278 bed days were provided to 24 individuals. FSL is the collaborating healthcare institution, which provides training and support to the therapeutic care family. The goal for every client is to maintain them in a home setting where they are incorporated into the family while preparing for permanent placement which could include a return to the family of origin, kinship care or independent living.
