Program areas at FSL Programs
Through its affiliates, foundation for senior living (FSL) has been making Arizona a safe place to age since 1974. FSL assists over 42,000 arizonans annually through more than 25 Programs centered around four primary areas: safe & affordable housing, nutritious meals, health & wellness, and caregiver support. In the fiscal year 2022-23, FSL continued its commitment to providing secure housing by offering shelter to over 1,500 arizonans each night. This encompassed various housing options, including single- and multi-family homes, affordable housing complexes, and residential behavioral health group homes. FSL also actively trained and monitored sponsors involved in adult foster care and adult behavioral health therapeutic home Programs. (continued on schedule o)as the largest provider of weatherization services in Arizona, FSL's weatherization and home improvement initiatives cut energy bills and improved health for over 8,000 homeowners. In the area of nutrition, FSL provided 200,000 meals to families in need via food pantries, congregate and home-delivered meal Programs. Health and wellness initiatives provided transitional care for over 900 patients upon hospital discharge and operated adult day health facilities, delivering over 150,000 hours of respite care to family caregivers. Throughout the fiscal year 2022-23, FSL remained steadfast in its mission, making significant strides in ensuring the safety, health, and well-being of arizonans of all ages.fsl home health agency was medicare-certified, licensed by the Arizona department of health services, and provided skilled services in the comfort of patients' homes beginning in 1985. In june of 2023, the home health program was discontinued due to difficulty staffing nurses and the abundance of other home health agencies in the area. In total during fye 2023, FSL home health agency provided 16,756 hours of skilled care to 12 patients. These patients required 24-hour supervision, and without the services provided by FSL, they would most likely be institutionalized. FSL maintained its medicare status providing a small number of intermittent visits during this time.
FSL's community living resources is a compilation of services designed to help ensure the provision of quality care in assisted living facilities across the entire state of Arizona as determined by the Arizona long term care services health plans and Arizona department of health services. These services include annual quality monitoring and periodic investigative audits of these facilities. For fye 2023, 404 quality monitoring audits were provided to ensure the residents and patients' health and well-being. Two of the three health plans chose to utilize their own internal staff to conduct monitoring visits beginning in 2022 which has reduced FSL monitoring services and staffing. FSL continues to contract with adult foster care sponsors to provide care services for mercy care members. In fye 2023, 21,514 safe bed nights were provided to under-resourced adults in the community.
In 1975, FSL opened the first adult day health services (adhs) center in Arizona to provide a safe, supportive and caring environment for seniors and adults with disabilities. FSL now operates two centers, branded as recreacin in glendale and tempe which are all licensed by the Arizona department of health services. Clients benefit from nursing services, personal care assistance and social service connections, all of which help them maintain their individual level of independence. 151,327 hours of care were provided to 386 clients in fye 2023. Along with providing supervision, FSL's dedicated staff create monthly therapeutic recreation calendars offering activities to meet the needs of FSL's clients' various cognitive and physical abilities. In addition to daily exercise Programs, educational offerings and community outings, 165,778 miles of transportation and 20,508 nutritious meals were provided to clients. Both of our recreacin locations resumed in person services and continue to focus on growing personnel and members back to pre-pandemic levels.
Care by design provides customized patient-centered care to support clients and caregivers with emotional support, community resources, sustainability planning and assistance with integration with FSL services. Together our social service team assisted 2,966 individuals to connect to care and supportive services during the fye 2023. In july 2020, the care by design team was contracted by wildfire to manage and distribute cares act, era and heaf funds to provide mortgage, rent and utility assistance for those who experienced financial hardships caused by the covid-19 pandemic. In fy2023, this team distributed $746,482. Activate (advance clients' transition to independence with actions that empower) provides transitional care services helping people to transition safely from hospital to home with a goal to improve health outcomes and to reduce re-hospitalization rates. Focusing on social determinants of health and education of disease processes and red flags, this program served 910 high-risk members of the community and provided complimentary access to 3,052 pieces of durable medical equipment to low-income patients. Clients experienced improved health and quality of life. FSL operates two community action Programs (caps), senior centers and food pantries in the peoria/nw valley and in wickenburg communities. In fye 2023 our sites distributed over 497,377 pounds of food, provided 5,942 rides to seniors, 68,047 meals through our congregate and home delivered meal Programs. The cap teams were contracted by maricopa county human services department and city of peoria to manage and distribute funds to provide mortgage, rent and utility assistance for those who experienced financial hardships caused by the covid-19 pandemic. Additional cap caseworkers were hired to manage these Programs and in fy2023, this team distributed $7,015,175.
