Program areas at Grand County Rural Health Network
The patient navigator program works in partnership with the client to coordinate healthcare, remove barriers to care, and educate on self-managed care. The patient navigator is a healthcare professional trained on navigation and patient empowerment models to assist the client in getting the care they need when they need it. In 2023, the program served at least 246 unique clients.
The partners for adults in need of services (p.a.i.n.s.) Is a program established by the Network and its partners to provide healthcare vouchers for acute, non-emergent medical and mental Health services to uninsured adults who qualify financially. In 2023, the p.a.i.n.s. Program served 96 unique clients via 282 vouchers.
Healthy Grand County is the collective impact model, with the Network as the backbone agency, to move the needle on key issues in our community, such as equity, mental Health, and affordable housing. The Health and human resources coalition, the partner group guiding healthy Grand County initiatives, is a collaborative of Health, human services, and government organizations working toward long-term, sustainable change to improve the Health of our community. The healthy Grand County program also promotes community engagement and equity initiatives, such as the community leader group. In 2023, more than 75 individuals participated in the community leader group.
The Health coverage guide program, in partnership with the northwest Colorado community Health partnership, provides free, in-person assistance and education for individuals and families enrolling in the state Health insurance marketplace. In 2023, the Health coverage guides assisted 221 households through 428 appointments. Advocacy for children's Health and education services (a.c.h.e.s.) Is a collaborative effort of Grand County agencies and community groups to promote the well-being of children. The aches program provides medical, dental and mental Health vouchers to uninsured children who qualify financially and are in need of acute, non-emergent care or preventative care. In 2023, the a.c.h.e.s. Program served 28 unique clients via 60 healthcare vouchers. The mental Health navigator (mhn) program is a collaborative between Grand County Rural Health Network, Grand County government, mind springs Health, middle park Health, winter park resort, and snow mountain ranch/ymca of the rockies. Master's level clinicians provide brief intervention to improve access to behavioral healthcare in primary care provider and other community settings, educate patients on the importance of mental Health services, and navigate patients to a long-term provider when possible. The mhn program has transferred to middle park Health for program sustainability. The gcrhn still leads the collaborative efforts, convening the oversight committee to ensure program goals and access remains patient-centered and provider-neutral.
