Program areas at Healthcare Collaborative of Greater Columbus
The Ohio dept of health- cdc ccr chw- engagement of all statewide pathways community hubs and their local partners. Hio works with odh and others to train, deploy and engage hubs and community health workers (chws) to prevent and find appropriate covid treatment (when necessary) as well as strengthen communities' resiliency to the pandemic via chws. Priority populations served by chws in this effort include pregnant women, minorities, older adults, adults with chronic diseases, individuals with developmental or intellectual delays and individuals with low socioeconomic status.
2023 city of Columbus summer youth rfp- program to introduce youth to community health concepts, resources, and skills of a community health worker. Students will also be exposed to the elements of trauma and how to use tools of de-escalation and regulation to combat violence.
The Ohio commission on minority health- ocmh-hio's- pathways community hub to deploy community health workers in identifying and supporting minority pregnant individuals in franklin and contiguous counties to ensure healthy, safe, prenatal, birth and postnatal care for parent and baby.
Support critical work to end childhood hunger through the integration of food Access into your state's sbhc services and improved connection of families to federal nutrition programs.
